Role of CGMS in Diabetes Management: Comparing Post-Meal Hyperglycemia Measured by CGMS and One Touch.

- Abstract
The objective of the study was to find if there was a difference in the post-meal hyperglycemia when measured by CGMS and one touch monitoring systems and if CGMS captured higher levels of post-meal hyperglycemia. To compare the measurements, retrospective chart review of 26 diabetic patients who visited endocrinology clinic was done. CGMS values and their corresponding one touch blood sugar values were collected from the patient's charts. Post-meal hyperglycemic episode was defined as the highest blood sugar value between the meals recorded by CGMS and one touch monitors. Meals were considered as breakfast, lunch and dinner. Both type I and type II diabetic patients were included in the study. Mean age of the patients was 58.19 years. Out of 26 patients, 14 were on insulin pump. Patients were connected to CGMS for an average of 64.93 hours. To find out if the difference between post-meal hyperglycemic values measured by CGMS and one touch was meaningful, paired t-test was conducted. Mean post-meal hyperglycemic value recorded by CGMS was 219.39 (SD = 75.77) and the mean value recorded by one touch, was 163.38 (SD = 75.19). Difference of 56.01 was found to be statistically significant [P=<0.001] with 95 percent confidence interval of 45.19 to 66.83. As indicated in the results, significant difference was found between the post-meal hyperglycemia recorded by CGMS and one touch blood sugar monitor. The spikes in postprandial blood sugar measured by CGMS, and at times missed by one touch meter, which translated into higher average post-meal hyperglycemia can be an important piece of information in the management of diabetes and strict glycemic control.
