- Abstract
Erectile dysfunction (ED) affects diabetic men three times as frequently as age-matched nondiabetic men and is associated with neurologic, vascular and biochemical changes in the male sexual organs as well as psychogenic influences. A total of 60 diabetic men with ED aged 40 and older participated in workshops consisting of five, two-hour meetings. The multidisciplinary team leading the workshops included a diabetologist, urologist, cardiologist, sex therapist and psychologist. All subjects were treated with sildenafil citrate and relaxation exercises, guided imagery and emotional therapy were conducted. Subjects completed a questionnaire on sexual behavior at baseline and at the final visit on which they rated their responses on a scale of 0 through 5, 5 being highest. At baseline, subjects rated their sexual desire significantly lower than at the end of the workshop: 2.8+/-1.3 vs. 3.2+/-1.2, p=0.04. Improved ease of maintaining an erection through to ejaculation was reported at the end of workshop, 2.3+/-1.6 vs. 2.97+/-1.6, p=0.02. Subjects' confidence in their ability to achieve and maintain an erection also improved compared to baseline: 2.0+/-1.0 vs. 2.7+/-0.9, p=0.01. Subjects also reported improvement in their ability to discuss sexuality with their partners: 2.89+/0.9 vs. 3.53+/-0.8, p=0.009. Though the effect of any of the components of the multidisciplinary workshop cannot be isolated, this study shows that a multidisciplinary approach to the complex problem of erectile dysfunction in diabetic men can improve patient sexual function.
