- Abstract
Introduction: Infantile hypertrophic pyloric stenosis (IHPS) in children is characterized by a progressive hypertrophy of pyloric muscle, the pathogenesis of which is not fully understood. There is increasing evidence suggesting that hypertrophy of the muscle and connective tissue remodeling is regulated by growth factors. Recent studies reported an increase in the local synthesis of some potent smooth muscle mitogens such as insulin-like growth factor-1 (IGF-1). However, there are no reports concerning local expression of platelet-derived endothelial cell growth factor (PDEGF), which plays an important role in the pathological angiogenesis. Aim of the study: The aim of this study was to analyse by immunohistochemistry (1HC) expression of IGF-I and PDEGF in muscularis propria of the hypertrophic pyloric muscle in children with infantile hypertrophic pyloric stenosis (IHPS). Material and methods: Full-thickness muscle-biopsy specimens were obtained from 22 infants at the time of pyloromyotomy. Control group consisted of 7 children undergoing surgery because of other alimentary tract defects. Paraffin tissue sections were evaluated by routine histopathological methods and by immunohistochemistry using monoclonal mouse anti PDEGF or IGF-I antibodies and polymer based peroxidase detections system. Morphometric analysis was performed using MultiScanBase V.808 analyzer. Ceils showing positive reaction were counted in 5 random 200x high-power fields. Values were expressed as the mean ±SD of the real expression area of the analyzed marker to the total analyzed area. Results: In children with IHPS the average area of PDEGF expression in the muscularis propria was 62 (±52.5) pkt/42 µm², whereas in the control group it was 15 (±12.1) pkt/42 µm². The average area of IGF-I expression in the pyloric muscularis propria was 1037 (±491.9) pkt/42 µm² in study group, whereas 259 (±221.44) pkt/42 µm² in the control group. Statistically significant differences in the expression of PDEGF and IGF-1 in the pyloric muscularis propria were found between the children with IHPS and the control group (p<0.05). Conclusions: These results show a local increase in expression of growth factors IGF-1 and PDEGF in the muscularis propria of the hypertrophic pyloric muscle in children with IHPS. These findings may have implications in the pathogenesis of IHPS.
