- Abstract
We currently lack systematic reviews answering the question whether second generation antipsychotics (SGA), compared with first generation antipsychotics (FGA), are more or less neuroprotective. An electronic literature search using MEDLINE (from 1966 until the end of January 2009) was performed. Retrieved clinical studies (218 publications containing 227 data sets) were classified into four categories: volumetric changes, neurotrophic factors, oxidative stress and neurotoxicity. Results showed more studies with SGA (72,5%) to have a positive impact on neuroprotective mechanisms than FGA (20,7%). Considering studies yielding negative results an opposite trend could be revealed: 13,7% versus 60,9%. Comparative studies were overwhelming in favour of SGA (57,1%) compared to FGA (2,4%). These lines of evidence re-emerged analysing the different sub-aspects of neuroprotection. This systematic review thus indicates that FGA and SGA may have different effects on neurotoxicity.
