- Abstract
Introduction: Extracorporeal shockwave lithotripsy (ESWL) is a commonly used procedure in urology. It is the preferred method of treating kidney stones<3 cm in size and has been shown to endure silent and often self-limiting acute and chronic lesions in the kidneys and adjacent organs. We conducted a randomized clinical trial to determine whether ESWL produces ischemia and reperfusion injury in the kidney and whether oral administration of antioxidant s reduces the degree of short-term renal injury in patients treated with ESWL. Methods: The study included 325 patients with renal stones (1-3cm in size) which were treated with ESWL. The patients were divided into two groups: Group A(n=150)which served as a control group and were not given any antioxidants. Group B (n=175) which were given a dose of Allopurinol (900mg) one and 8 hours after ESWL. Urine samples were obtained from all patients just before the start of the treatment with ESWL and at 2 and 24 hours, and on the 5th and 15th day after ESWL urinary levels of β2 microglobulin and severity of hematuria determined as measures of renal injury. Results: These findings suggest that treatment with ESWL generates free radicals through ischemic reperfusion injury mechanism and that oral administration of antioxidant may protect these patients from short-term renal injury caused by ESWL. Conclusions: These findings suggest that treatment with ESWL generates free radicals through ischemic reperfusion injury mechanism and that oral administration of antioxidant may protect these patients from short-term renal injury caused by ESWL.
