Role of Inflammatory Mediators in Idiopathic Generalized Tonic Clonic Epilepsy.

- Abstract
Background: The role of inflammatory cytokines in the epileptogenic process is elusive. Seizure activity can induce an inflammatory response by activation of microglia and release of cytokines. In addition, some cytokines have been implicated in the generation of the epileptic activity. Objective: to analyze cytokines levels in patients with idiopathic generalized tonic- clonic epilepsy and correlated it with various parameters. Methods: Serum and cerebrospinal fluid (CSF) samples were clollected from 45 volunteers and 45 patients with idiopathic generalized tonic clonic epilepsy during the immediate post-ictal phase, a second serum sample was collected from patients during the interictal phase. The serum levels of TNF-α and IL-2 and the CSF level of IL-6 were assessed using ELISA. Results: As compared to controls, the patients showed elevated serum post ictal IL- 2 and TNF-α levels (P<0.001). Analysis during the interictal phase revealed a decrease in serum cytokine levels relative to the postictal phase, but still was higher than control (P<0.001& 0.03 respectively). CSF post ictal IL-6 was higher in patients (P<0.001). A positive correlation was found between postictal serum TNF-α level and seizure frequency (P <0.001). Postictal serum TNF-α level was higher in patients on polytherapy than those on monotherapy (P=0.012). Conclusion: Increased levels of inflammatory cytokines IL-2, TNF-α and IL6 in patients with idiopathic generalized tonic clonic epilepsy may reflect a postictal sequel, in addition may point to a possible role in the generation of the epileptogenic activity itself.
