- Abstract
Objective- Background We report a case of Veillonella dispar causing an extensive spinal epidural abscess with discitis, a left psoas abscess and bacteremia. Veillonella is an anaerobic gram negative coccus present in the upper airways, gastrointestinal tract and vagina in humans and usually disregarded as a contaminant when isolated from clinical specimens. Veillonella dispar has previously been described as a cause of endocarditis and prosthetic joint infection. Case Presentation An obese male presented to the emergency department with a history of multiple falls, unsteady gait and worsening mobility. During hospitalization he developed new neurological symptoms involving all four limbs. Magnetic resonance imaging showed an extensive spinal epidural abscess, discitis and a psoas abscess. Veillonella dispar was isolated from three sets of blood cultures. The patient was treated conservatively with long term antibiotics and made a near full recovery. Discussion We emphasize that a rare gram negative bacteria when isolated repeatedly from blood cultures requires appropriate investigation to look for a deep seated source. ?n a patient with significant co-morbidity, an extensive spinal epidural abscess and discitis could be managed non-surgically. To the best of our knowledge this is the first case report of Veillonella dispar causing extensive deep seated abscesses, discitis and bacteremia.
